noteOff missing

I'm trying to develop some midi code for an iPhone app and I've notices something that seems strange about the Gaia when used as a keyboard controller. I'm just learning about MIDI now so what I'm seeing might be perfectly normal. The scenario is this:
I press one key on the gaia and while holding the key down, press another. I release both keys simultaneously. In the midi stream I see a note on message for both keys and then a single note off message for one of the keys and then another note on for the other key. This new note on has a non zero velocity. Where did the other note off go and why is there another note on message?
If anyone can explain this to me I would appreciate it.
